What are your hours are you open?

Buffalo Spine Surgery is open Monday through Friday from 8:00 a.m. to 4:00 p.m.
Dr. Cappuccino sees patients in our Ellicottville office one Friday a month.

What should I bring to my first appointment?

At the time of your initial appointment please bring:
  • Photo ID
  • All insurance cards
  • A valid referral
  • Co-Pay
  • A fully completed patient information sheets
  • Any x-rays, CTs, MRIs (actual films and reports)
  • Exact dates of injury

What should I bring to follow up visits?

At the time of follow-up visits please bring:
  • Insurance cards
  • Co-pays
  • Updated referrals
  • Any studies
  • An updated patient information sheet every 6 months

Can I request refills to prescriptions by phone?

Requests for prescriptions must be made between 9:30 -3:00 Monday through Thursday. The prescriptions will be called in or available for pick up (narcotics) within 24 hours. Please try to anticipate the need for refills when you see your medication is running low.
Narcotics will be prescribed by Dr. Cappuccino in the immediate post-operative period only (no more than 6 weeks after surgery). Chronic pain requiring more protracted courses of narcotics is best-managed by a specialist trained to deal with chronic pain. We will coordinate a visit to a pain management specialist for you.
Narcotics will not (under any circumstances) be reordered during weekend or night on call hours.
Requests for narcotic refills must be called to the office, and the prescription can then be picked up. Refills will not be done by phone.

If I am having trouble, can I get in touch with the doctor after business hours?

After hours care is available for acute and urgent problems. Our team of PAs, NPs and doctors will be available on a rotating basis to answer after-hour calls. A physician will be available as necessary for medical and surgical emergencies.

Do you accept Medicare?

Yes. However, we cannot accept assignment on Medicare. You are responsible for full payment for services rendered. We will submit your claims for you. Please be aware that Medicare only covers 80% of your total bill.

Do you accept Worker's Compensation or No-fault?

In order to submit claims for you, you must provide us with information as outlined in our new patient forms. We will need your carrier name, address, and phone number, date of accident/injury, exact circumstances, and your claim number. If you do not have this information available you must either assume full responsibility for payment, or reschedule the appointment. In the event the insurance declines or terminates your benefits, or you use up your allowance, you will be responsible for full payment for services rendered.
